Pavel (@zheln) • Hey
A doctor’s Lens.
Publications
- Probiotics won’t make you poop, but lactulose will, with moderate certainty. <https://doi.org/10.1007/s00394-024-03410-1>
Taking lactulose for an average of 4 weeks leads to at least one more additional stool per week in adults with positive Rome criteria for constipation. <https://www.mdcalc.com/calc/10003/rome-iv-diagnostic-criteria-functional-constipation>
- Dipeptidyl peptidase-4 (DPP4) inhibitors do not work. https://doi.org/10.7326/M23-1490
The market share is US$ 11.6 billion. https://www.mordorintelligence.com/industry-reports/dipeptide-peptidase-4-inhibitors-market
They are not effective at all, with high certainty.
DPP4 inhibitors are one of the newer drugs people with type 2 diabetes use to lower their blood sugar levels. They include medications such as Januvia (sitagliptin), Onglyza (saxagliptin), Tradjenta (linagliptin), Nesina or Vipidia (alogliptin), and Galvus (vildagliptin).
Objections?
- Omni is the new norm.
-
- False fire alarms. Is that a frequent thing where you live? Do you think they improve fire preparedness?
- Why are we living in the age of tokens? Here is why:
1. Tokenistic policies
2. Crypto tokens
3. Text tokenization
How many hype issues of today can be covered by just these three?

- In health research, a hazard is not a risk. Now check this out:
“This study aimed to investigate the association between hypertensive disorders of pregnancy (HDP) and subsequent risk of dementia using a systematic review and meta-analysis of cohort studies. \[…] Using the random-effects model, pooled hazard ratio (HR) and 95% confidence interval (CI) of dementia risk in women with HDP were calculated.” (citation below)
If individuals with hypertensive disorders of pregnancy have a higher hazard of developing dementia, that means they are progressing toward it *faster*, not that more of them develop it eventually. That’s because we don’t know how many will develop it eventually because researchers don’t usually follow participants for decades, so they censor them at the last available follow-up, and then they use the hazard function to account for censoring. Yet, an article consistently confusing the two is getting published in a Nature journal. I had to remove the word fucking many times when I typed.
The search strategy was also a shame. Just take a look at its major dementia-related part:
("dementia"\[MeSH Terms] OR "dementia"\[All Fields] OR "dementias"\[All Fields] OR "dementia s"\[All Fields])
Just so you know, a \[random Cochrane review on dementia]\(<https://doi.org/10.1002/14651858.CD013388.pub2)> features **dozens** of synonyms of dementia in the search strategy, so one could wonder how this impacted the number of studies found.
To sum up the mess, there goes a risk of bias assessment using the Newcastle-Ottawa scale. Although I know that many folks still use it, I do urge everyone to acknowledge the existence of \[ROBINS-E]\(<https://doi.org/10.1016/j.envint.2024.108602)> in 2024.

- Okay, @club/orb correct me if I’m wrong. Here is a link to the Polygon transaction that created a contract for one of my recent publications: <https://polygonscan.com/tx/0x1bb38f0731527be860ce00ecdcf8c9af64217f7862aa2c8fcca01ae378884e9b>
From the tx details, we can get my @lens/lens profile # and the publication # (apparently incremented with each new one on this profile). However, there is no link to the actual publication creation tx, which is this: <https://polygonscan.com/tx/0x46f25855c5b4e45f4ae2429b6462aed1fdba96698db8f7ca7a4c4d0a8ed0e4f2>
So, does it mean that there is no way to actually get from the collectible’s NFT on the blockchain to the actual post content w/o leveraging the Lens infrastructure (i.e., the profile and publication numbers)? Does it make sense, anyone?
- I really like that summary of findings table, haha.

- Not sure where this fits in (I didn’t find @club/medicine or @club/health, only @club/mentalhealth but that’s not *exactly* it, and I also don’t know how to create a new Orb 'club') – just wanted to share a core outcome set for opioid use disorder if you need one.
An outcome set is basically a list of metrics researchers, health professionals, policy-makers, informed patients, carers, and other health knowledge users are invited to turn to when interpreting the results of a research study on a given topic or looking to do one.
"…consensus was reached for five outcomes: two patient-reported (global impression of improvement and incident non-fatal overdose); one clinician-reported (illicit/non-medical drug toxicology); and two from administrative records (duration of treatment and fatal opioid poisoning)"
In summary, you now have a set of metrics to look at when talking about the potential health benefits of treatment or prevention strategies for opioid use disorder.

P.S. I wish I could just post a semantic data file on Lens instead of plain text! Hoping to get a dapp supporting that some day.

- It’s such a good feeling to post and know that your post is gonna be stored on a blockchain. I still have some concerns, however. For instance, that Giphy – it uses neither Arweave nor IPFS, so the link to a gif might as well get broken with time. Also, I’m looking forward to the implementation of an L3->L2 bridge by @lens/lens devs.
