what do the 3 clusters separated from the main central cluster represent? are radius from center and degrees representative of a real world metric?

Great questions! There's no meaning to the angular position of the points. There's also no meaning to the absolute distance from the center other than that the most connected profiles tend to be toward the center. The way these graphs are laid out is that points that are connected via a follow attract each other and points that aren't repel each other. These simple rules allow the structure present in the graph to reveal itself. The smaller clusters that you see are groups of users that follow each other more than they follow the rest of the graph.
